For many years, 60% of the facelifts Dr. Buchanan did had already had a facelift but had come in to eliminate the re-aging that had occurred since the procedure. Another 5-8% were under 50 years of age. Almost all of these had very little problem in the neck, but, at the time, a surgical procedure was the only alternative to achieve a return to a youthful appearance. Today both groups can be rejuvenated exceptionally well without surgery.
Others have jokingly called the use of fillers such as Juvéderm® Ultra, Juvéderm® Voluma, Juvéderm® Volbella and Juvederm® Vollure a “Liquid Facelift” or “Facelift in a Syringe” because they can almost give you results similar to a facelift. We call our approach “Today’s Liquid Lift™” because we go the next step and include Botox® and fat injection in the methods we use to achieve a more relaxed, natural and longer lasting result. We consider more than just the lines around the mouth or between the eyes. Instead, we concentrate on what is causing you the aged appearance and treat what will give you the most optimal reversal of the problem with the least expense and downtime. We also use these methods to correct nasal deformities (what some others call a Liquid Rhinoplasty) or improve the aged appearance of the hands.
Today’s Liquid Lift™ is especially useful for early changes of aging where the majority of the changes are from deflation of the tissues rather than significant loose skin. The deflation causes what appears to be loose skin and folds. As an example, nasolabial folds (those creases from the nose down and outward) are caused by loss of fill in the malar (or suborbital) area with the descent of the overlying skin. For a number of years since fillers first became available these have been treated by injecting the filler under the fold to efface it. This, however, did nothing for the aging above the fold and, in fact, worsened the aging by enhancing the squareness of the face rather than restoring the triangular appearance of youth. Modern technique instead refills the malar area, correcting the aging there, and also elevating and reducing the nasolabial fold. Similar correction of early jowling and lateral lip commissural folding can also be achieved by filling over the malar and zygomatic areas. Tear troughs under the eyes have been treated by direct injection of a filler. This, however, risks significant problems including swelling. Instead, we now inject over the malar bone below the orbital rim to elevate the cheek tissues, thus eliminating the tear trough. This technique is now tought by Allergan the maker of Juvéderm® for use with Voluma™, though we had been using it with Radiesse® since 2005. We switched to Voluma® when it came out since it lasted longer and was potentially safer.
